I (We) agree to accept responsibility for any and all events that occur in connection with the fostering of a rescue. I (We) agree to release, indemnify, and hold harmless, Saint Croix Animal Friends (SCAF) , its agents, members, officers, volunteers or employees, from any and all claims, known and unknown, now or hereafter, arising in connection with the rescue cat/dog.
I (We) also understand that many of these cats and dogs have lived in a neglectful and/or abusive environment, and have probably lost their self-esteem and confidence. I (We) understand that this cat/dog will need rehabilitating to become the warm, loving companion every cat/dog should be. I (We) agree to provide the loving environment, the socialization, and the training that this cat/dog will need to become a beloved member of a family.
I (We) agree to comply with all state and local laws and ordinances relating to the temporary fostering of this cat/dog.
I (We) understand that the rescue has been fully evaluated by a veterinarian. Excepting a bona fide emergency, I (We) agree to obtain prior approval from Saint Croix Animal Friends for any additional medical attention believed necessary. I (We) agree that any medical treatments that do not have prior approval from Saint Croix Animal Friends are my financial responsibility.
I (We) agree to transition the rescue into his adoptive home as quickly and as successfully as possible. I (We) will complete an evaluation of the dog’s personality, behavior and temperament and provide the foster chair with a copy of this assessment.
I (We) certify that the above information is true and accurate to the best of my (our) knowledge, and I (We) understand that completion of this form in no way guarantees my ability to foster a dog.
I (We) further understand that completion of this form is only the first step in the foster process and that, should I (We) wish to further this process, and in-home screening will follow.
